Join Us for the 200,000 About Me Conversations Campaign

The About Me Conversations Campaign is an initiative to have “About Me” conversations in 200,000 medical or social care conversations by the end of 2025. You can participate as an individual, an advocate, researcher, clinician, social worker, or institution.

Our commitment to you and the public:

  • Over the course of the campaign we will send you a newsletter highlighting the “About Me” stories people choose to share and the impact of the conversations on the well-being of participants.

  • You may list your name or the name of your organization as a participant in the Campaign on the Institute's website.

  • At the end of the campaign (in 2026), we will host a conference to share stories about the impact of the “About Me” conversations and what we can do next to continue advancing care based on the goals, preferences, and circumstances of every patient (person).

Adopting Care Practitioner (Clinician, Social Care Worker, Care Advocate)

^We’ll provide you suggestions for implementing in your workflow and facilitating conversations using the Card.

Organization or Community Partner (Funder, Research or Civic Org, Health System)

Click below to commit to being an organization recognized for spreading the adoption and impact of the About Me Card.#

^We’ll partner to conduct research, provide the About Me Card to community residents, raise funding for scaling the Cared, or any other ideas you may have.
