Clinical Practice or Health System Implementation of An About Me” Care Card & Care Model Implementation: Let’s partner to train your leaders and staff how to Design, Implement, Measure & Continually Improve your own “About Me” Care Model. We’ve have program implementation “kits” we can use to methodically implement a Card and care model based on the local context. We can successfully implement the About Me Card and care model in any of the following settings:

A single hospital or multi-hospital health system

Creating New About Me Cards for a specific patient population: Partner with use to lead an initiative focused on creating a new About Me Card for a specific patient population. We recently designed and led a global initiative that resulted in the “About Me Card for the Aging Population.” The Aging About Me Card was implemented in Epic within 3 months of the project’s completion.
